Pulp canal obliteration (PCO) in calcified teeth presents significant challenges for endodontic therapy, especially when associated with discoloration. This case report illustrates the successful management of a discolored maxillary central incisor exhibiting PCO through guided endodontic access (GEA). A 28-year-old male with a history of dental trauma presented with discoloration with tooth #11 and a missing tooth #21. Clinical examination revealed a non-vital tooth with asymptomatic apical periodontitis. A three-dimensional (3D) treatment plan was utilized to create a 3D template by cone-beam computed tomography and intraoral scanning, enabled minimally invasive palatal access. Endodontic therapy was completed in a single session. Subsequent prosthetic rehabilitation was planned for the missing left central incisor. This case highlights the efficacy of innovative GEA as a conservative and predictable approach to complex cases involving calcified canals, discoloration, and missing teeth, reducing the risk of iatrogenic errors while preserving tooth structure and surrounding periodontal tissues through reduced heat generation with high-speed airotor compared to conventional micromotor-based access cavity preparation facilitating esthetic restoration.
